This screening instrument is designed to open up a discussion. Patients are asked to complete both Part A and Part B of the Symptom Checklist by marking an X in the box that most closely represents the frequency of occurrence of each of the symptoms. Filling out the form should open up a discussion of the history of ADHD symptoms and the degree that which the symptoms interfere with day-to-day activities.
