Receipt For Therapeutic Services

This form can be given to a client as a receipt for services. There is one receipt for monthly services and one for individual sessions.

Choosing Abstinence or Moderation

This worksheet is designed to help people with addictions determine whether they will be more successful in moderating their addictive behaviors or completely abstaining. The worksheet includes a rating scale of factors that might make it harder for people to moderate their addictions, as well as questions to discuss with a therapist or counselor. This checklist is designed to help therapists ensure they have “covered all the bases” with new clients.

This generic treatment plan can be modified and used to document client objectives, strategies, and methods of assessment.

General Office Policies

This form is designed for therapists to give to new clients. It states general office policies, such as waiting room and building rules, payment policies, policies for contacting the therapist, and so on.

Sliding Scale Fee Agreement

This form can be modified to state a therapist’s sliding scale schedule and to obtain a signed agreement regarding the terms of a reduced fee.

Developmental History Form

This form can be used to obtain or release confidential information.

Life History Questionnaire

This questionnaire covers a wide range of issue that would be useful as a starting point in therapy, including a checklist of problems, information about family background, health issues, and so on. Type: PDF (1014)

Drug Testing Consent Form

This form can be used to obtain consent for drug testing. The form should be modified to fit the needs of the individual therapist or clinic.
